- a valid passport
- a filled out application form. Please fill out the application online, print and sign before sending. Adobe Acrobat required.
- one passport size photo (2" x 2")
- a prepaid FedEx waybill (if you want your visa couriered back to you)
- certify cheque or money order payable to Eurosvit Travel Ltd. for the specified amount, depending on visa type
